This transaction is equally secure to placing an on-line order or phone order. If you have any comments or if there is not enough space on the form, feel free to attach multiple sheets. Please use our website as a reference when placing your order. You can find the Item number (product code) in the product description next to the price. Product codes start with 1-4 letters and are usually followed by a series of numbers. When calculating shipping costs, please refer to the Shipping Info section of our site.
